Frequently Asked Questions About Direct Insurance Billing

What Should You Bring With You?

  1. Your extended benefits card, including the member ID and policy number
  2. A doctor’s reference, if your policy requires one

How Do I Find Out How Much Insurance I Have?

You can get more information about your coverage by getting in touch with your extended health provider or your plan administrator.

What If My Extended Health Insurance Only Covers A Portion Of My Visit?

Even if your extended health insurance only partially covers your visit, we can still charge your carrier.  If your visit is only partially covered, we will bill the covered portion to your insurance company and ask you to pay the remaining balance at the conclusion of each treatment.

Is It Possible To Coordinate Benefits?

Reform PT Burnaby will only bill one third party insurance provider per appointment at this time (eg: 1 Extended Health Carrier or ICBC or MSP). Unfortunately, benefits coordination is not possible.

Is It Necessary For Me To Have A Physician's Referral?

Although some insurance plans do require a doctor’s referral, Reform PT in Burnaby does not require a recommendation from a physician. For more information on your plan’s criteria, please contact your extended health carrier.

What If I Have A Pending Insurance Claim With ICBC, Worksafe, Or Another Company?

We cannot bill your visit to your extended health carrier if it is connected to an open insurance claim, if you have one. The cost of the treatment will be your responsibility; and in this case you can ask your insurer for reimbursement.

What Do I Do If My Claim Is Rejected?

If this is the case, we will ask that you pay the remaining payment in full at the conclusion of each treatment. If your visit is not approved for online direct billing, before you leave, the front desk staff will let you know whether your claim has been rejected. Please be aware that many plans have restrictions that prevent online direct billing. Please get in touch with your extended health carrier to find out if you qualify for this service.
